Yes, the first time I caned someone for real was nerve-racking (and a deeply powerful experience both ways). I was incredibly fortunate that it was with someone where the trust and level of caring was mutual. Was I good at it? Put it this way, I'm sure my skill levels have improved considerably with practice!
